Senior CommUnity Care of Michigan PACE is a comprehensive health care choice for seniors living in Ingham, Eaton and Clinton counties and bordering areas of adjacent counties. With Senior CommUnity Care of Michigan, PACE stands for Program of All-Inclusive Care for the Elderly.

Senior CommUnity Care of Michigan is for:

As a PACE provider, Senior CommUnity Care of Michigan helps people live independently at home with a high quality of life. Senior CommUnity Care of Michigan is a service of Volunteers of America. VOA is a respected nonprofit, faith-based organization. Nationwide, VOA helps nearly 2 million people every year through health services and programs for children, teens, adults and entire families.

Mission

Our mission is to provide health care services and support for elders living in Ingham, Eaton and Clinton counties and select zip codes of adjacent counties. This will help seniors stay independent, healthy, connected with friends and family in the community, and living at home.

Our Team

The Senior CommUnity Care of Michigan team includes health care professionals who know how to manage the care of the elderly. The team creates a personal health care plan for every participant. This plan will help monitor changes in health care needs. It also helps manage all health care services and provide support for participants in the home and our Center. The team includes:

  • Doctors.
  • Nurses.
  • Personal care providers.
  • Home health providers.
  • PACE Center Manager.
  • Occupational and physical therapists.
  • Social workers.
  • Activity staff.
  • Dietitians.
  • Drivers.
  • Spiritual counselors, if needed.
